In addition to evaluating symptoms, interpreting labs, and treating STIs in a timely manner, the Nurse Practitioner plays a key role in PrEP care- including same day PrEP starts when appropriate- through risk assessment, patient education, prescribing, follow-up planning, and coordination with the Wellness team and AHF Pharmacy to support initiation and retention.
The Nurse Practitioner also supports our Rapid ART (Rapid Start) model by initiating antiretroviral therapy (ART) the same day a client is newly diagnosed with HIV (as clinically indicated ), ordering baseline labs, and ensuring streamlined results review and follow-up within 24-48 hours.
The Nurse Practitioner coordinates a warm handoff and timely transition to HIV primary care at an AHF Healthcare Center (HCC) so clients remain safely bridged from the starter pack to their first comprehensive HIV care visit. You will be a passionate advocate for our top initiaties.
