Key Responsibilities
The RN Case Manager provides hands-on hospice nursing care and coordinates services for patients and families within the community. They collaborate with the Clinical Director to assess eligibility and ensure the delivery of compassionate, high-quality end-of-life care.
Requirements
Candidates must hold a current, unencumbered Registered Nurse license and have at least one year of experience in a medical-surgical or acute care setting. Proficiency in EMR systems, a valid driver's license, and current CPR certification are also required.
