Licensed Practical Nurse Case Manager

We are seeking an LPN Case Manager to join our team in our Crafton, PA location.

Earn $29 per hour with benefits or $31 per hour with the selection of enhanced pay and scaled back benefits.

Schedule: M - F 8:30 AM - 9:00 PM

Position Details

• Responsibility and accountability for the overall coordination of the medical portion of the plan of care and services for a designated caseload of residents
• Utilization of a collaborative approach by the NCM with all members of the health care team will ensure the plan implementation
• NCMs will function as the nursing liaison across disciplines within the AVS system, with family members/caregivers, and with community-based health care providers/agencies to ensure medical and nursing care is appropriate and timely.

Merakey AVS is a non-profit organization which provides homes and a full range of services to adults with all levels of intellectual and developmental disabilities (IDD). The majority of the nearly 800 individuals that we support are diagnosed with severe or profound IDD. In addition, most of the people we serve have multiple physical disabilities, extensive medical complications and may require supports for challenging behaviors.

Qualifications

The ideal candidate will possess the following qualifications:

• Current license as an LPN in the Commonwealth of PA.
• Preferred two years of hospital experience.
• Genuine interest in working with individuals with physical and developmental disabilities.
