Job Description

Make a meaningful difference—one patient, one home, and one family at a time. Adoray Home Health and Hospice is seeking a full-time Physical Therapist to join our compassionate Home Health team, with a $5,000 sign-on bonus available for this role.

Requirements

Minimum Qualifications

  • Graduate of a physical therapy program with curriculum approved by the American Physical Therapy Association or equivalent (as outlined in the Federal Register defining qualifications).
  • Current Wisconsin Physical Therapist license required.
  • Minimum of two years of clinical experience preferred.
  • Excellent communication skills.
  • Working knowledge of community resources.
  • Working knowledge of Home Health and Hospice.
  • Ability to work autonomously and make independent judgments.
  • Ability to creatively solve problems.
  • Evidence of attendance of hospice training class or willingness to attend.
  • Basic computer skills.

Key Responsibilities

Provide physical therapy services to patients in their homes to improve their quality of life. Collaborate with a compassionate team to ensure patients remain safely and comfortably in their own residences.
