Key Responsibilities
The RN Case Manager performs comprehensive patient assessments and implements individualized plans of care for patients with life-limiting illnesses. They collaborate with interdisciplinary teams and physicians to ensure coordinated care and regulatory compliance.
Requirements
Candidates must hold a current State Registered Nurse license and have graduated from an accredited nursing program. A minimum of two years of clinical RN experience is preferred, along with a valid driver's license and auto insurance.
