Key Responsibilities
The RN Case Manager performs comprehensive physical and psychosocial assessments to implement individualized plans of care for patients with terminal illnesses. They collaborate with interdisciplinary teams and physicians to ensure coordinated care and compliance with regulatory standards.
Requirements
Candidates must hold a current State Registered Nurse license and have graduated from an accredited nursing program. A minimum of two years of clinical experience is preferred, along with a valid driver's license and auto insurance.
