We are seeking two Physical Therapists to join our Health in Motion team, with both full-time and part-time opportunities available.

Position Summary Pagosa Springs Medical Center (PSMC) is seeking two Physical Therapists to join our Physical Therapy team. We are open to candidates interested in either full-time or part-time employment.

The Physical Therapist works with patients experiencing disabilities, disorders, injuries, chronic conditions, and other limitations affecting movement and function.

The PT evaluates patients, develops individualized treatment plans based on functional goals, provides therapeutic interventions, monitors progress, and modifies treatment plans as appropriate.

Key Responsibilities

Patient Evaluation & Treatment Evaluate patients and interpret assessment and test results to determine appropriate physical therapy treatment plans Develop individualized treatment plans that identify patient goals and expected outcomes Provide physical therapy interventions designed to improve mobility, restore function, and relieve pain Utilize appropriate therapeutic techniques and modalities, including exercise, stretching, manual therapy, massage, heat, water, and other approved treatments Provide gait and balance training Fit and instruct patients in the appropriate use of braces, prostheses, crutches, walkers, wheelchairs, and other supportive or assistive devices Assist patients with transfers and movement to and from treatment areas and therapy equipment Measure and assess range of motion, vital signs, and other appropriate clinical indicators Evaluate patient progress and modify treatment plans as necessary Patient Education & Continuity of Care Educate and encourage patients throughout the rehabilitation process Develop home exercise programs and provide instruction to patients, families, and caregivers Reinforce education regarding treatment plans, physical challenges, recovery, and physician instructions Recommend appropriate assistive equipment and follow-up services Participate in discharge planning in collaboration with physicians, nurses, social workers, and other healthcare professionals Help ensure continuity of the therapeutic plan following discharge Documentation & Care Coordination Accurately and efficiently document initial assessments, treatment provided, patient progress, and changes to treatment plans in the electronic health record Review orders and patient records in preparation for treatment Verify appropriate orders, medical necessity, and insurance authorization before treatment Collect and document patient-related quality data in accordance with departmental workflows and protocols Coordinate patient care with providers and other members of the healthcare team Respond appropriately to patient communications and portal messages Ensure appropriate follow-up appointments are coordinated when needed Quality & Patient Safety Practice within the Physical Therapist scope of care as defined by the State of Colorado Utilize appropriate patient identifiers and follow National Patient Safety Goals Perform patient care in a manner that promotes safety and reduces the likelihood of healthcare errors Follow appropriate infection prevention and universal precaution practices Recognize and appropriately address the psychosocial, cultural, religious, developmental, and age-specific needs of patients Maintain current knowledge of best practices and evidence-based physical therapy treatment Identify and address potential concerns related to patient care or supporting activities Department Operations & Collaboration Work collaboratively with providers, employees, patients, leadership, and other members of the healthcare team Communicate effectively and keep appropriate team members informed and involved in patient care Maintain treatment areas, therapy equipment, and necessary supplies Support proper operation and maintenance of physical therapy equipment Participate in departmental activities, education, and training Support the development of the Physical Therapy team through information sharing and in-service education Demonstrate sound judgment, adaptability, critical thinking, and professionalism in a fast-paced healthcare environment Qualifications: Education & Licensure Graduate degree in Physical Therapy and/or Doctor of Physical Therapy (DPT) degree from a physical therapy program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE) Successful completion of the Federation of State Boards of Physical Therapy (FSBPT) National Physical Therapy Examination Current Physical Therapist licensure in the State of Colorado Current AHA BLS certification for healthcare providers Certification in the treatment of lymphedema is strongly desired but not required Experience Two (2) years of experience practicing as a Physical Therapist is strongly desired but not required This provides an opportunity for both experienced Physical Therapists and qualified therapists who may be earlier in their careers.

Physical Therapists work in hospital and outpatient settings and have frequent interaction with patients, employees, providers, vendors, and the public.

The position requires the physical ability to assist with patient movement, positioning, transfers, and therapy activities and is classified as involving heavy work under the physical demands identified in the job description.

Compensation & Benefits Base

Compensation

Range: $43.12 - $53.90 hourly Benefits: Full-time Benefits include generous paid time off, separate sick leave, health, dental, vision, life and AD&D insurance, long-term disability and option for short-term disability, and retirement plan with employer contribution.
