Make a Meaningful Impact in Hospice Care
Signature Eugene Home Health is seeking a compassionate and dedicated Social Worker to join our interdisciplinary team. This role is ideal for a social worker passionate about psychosocial support, and patient advocacy.
As a Home Health Social Worker, you will provide critical emotional support, care coordination, and community resource guidance to patients and families navigating life’s most meaningful transitions.
- What You’ll Do (Key Responsibilities)
- Conduct comprehensive psychosocial assessments for patients and families
- Develop and implement care plans based on social and emotional needs
- Provide grief counseling, bereavement support, and crisis intervention
- Coordinate community resources, referrals, and support services
- Serve as a liaison between patients, families, and healthcare providers
- Collaborate with the interdisciplinary home health care team (RN, physician, aides)
- Participate in quality assurance and performance improvement initiatives
What You Bring
- Licensed in Social Work (LCSW) from an accredited program
- 1+ year of healthcare experience (hospice or home health preferred)
- Strong communication, organization, and interpersonal skills
- Ability to work independently in a home health / field-based setting
- Current CPR certification
- Valid driver’s license and reliable transportation
