Job Description
Physical Therapist, Home Health
We're seeking a compassionate and skilled PT to join our home health team.
This full-time position offers the unique opportunity to provide personalized care to our diverse client base, helping them achieve their health and mobility goals.
Position Responsibilities
Provides patient care and documents treatment in accordance with physician orders, agency policy and patient needs.
Informs the physician of the results of the assessment and recommended clinical interventions prior to initiating care.
Develops and documents appropriate treatment plans for patients including selecting appropriate intervention approaches, strategies, materials and establishing treatment priorities.
Provides an organized and efficient patient treatment which includes advising, educating and counseling the patient.
Presents pertinent patient information to physicians and keeps team members informed of patient status and treatment plan.
Provides appropriate treatment to patients according to patient needs and within the parameters set by the agency and payer.
- Minimum nine months experience as a physical therapist
- Preferred community/home health experience
- Master’s degree in physical therapy approved by CAPTE accredited physical therapist education program
- Must possess and maintain valid CPR certification while employed in a clinical role
- Must be a licensed driver who can travel to all business locations

Requirements
Candidates must hold a Master's degree in physical therapy from a CAPTE-accredited program and possess at least nine months of experience. Additionally, a valid CPR certification and a driver's license for travel to business locations are required.
