Per Diem Home Care - Occupational Therapist

Key Responsibilities

The Occupational Therapist will assess, plan, and implement therapy modalities to improve patient physical function and independence. They will also collaborate with the care team, consult with physicians, and maintain accurate clinical documentation.

Requirements

Candidates must be graduates of an accredited Occupational Therapy program and hold an active Minnesota license. A minimum of four years of experience is required, along with current CPR certification and a valid driver's license.
