Home Health Assistant Director of Nursing

The Assistant Director of Nursing (ADON) supports the Director of Nursing (DON) in overseeing all clinical operations for SA Nurses Home Health Agency. This role ensures high‑quality patient care, regulatory compliance, staff performance, and smooth day‑to‑day clinical workflow. The ADON serves as a clinical leader, mentor, and resource for field nurses, case managers, and interdisciplinary team members.

Key Responsibilities

Support the DON in managing daily clinical operations and ensuring compliance with state, federal, and agency standards.

• Assist with supervision of RNs, LVNs, CNAs, and other clinical staff.
• Review, approve, and monitor clinical documentation, including OASIS, plans of care, and skilled visit notes.
• Participate in case conferences, chart audits, and quality assurance/performance improvement (QAPI) activities.
• Ensure timely completion of assessments, recertifications, discharges, and care‑plan updates.
• Provide clinical guidance to field staff regarding wound care, medication management, disease processes, and patient safety.
• Support onboarding, training, and competency validation for new clinical staff.
• Assist with scheduling, staffing coverage, and managing patient assignments.
• Communicate effectively with physicians, hospitals, and interdisciplinary team members to coordinate patient care.
• Participate in investigations related to incidents, complaints, or performance concerns.
• Ensure adherence to agency policies, infection‑control standards, and best‑practice clinical guidelines.
• Serve as acting DON when needed.

Qualifications

• Active RN license in the state of Texas.
• Minimum 2–3 years of home‑health experience; leadership or supervisory experience strongly preferred.
• Strong knowledge of OASIS, Medicare Conditions of Participation, and home‑health regulations.
• Excellent communication, leadership, and problem‑solving skills.
• Ability to manage multiple priorities and support staff in a fast‑paced environment.
• Proficiency with EMR systems (WellSky/Kinnser or HCHB preferred).
