Requirements

• As our PT, you’ll evaluate patients and build individualized care plans, then adjust them as progress unfolds.

• You’ll perform skilled therapy in patients’ homes, keep physicians in the loop on progress and changes, and guide patients and families through home exercise programs.

• You’ll also supervise PTAs and HHAs as needed, stay current on documentation (OASIS, progress notes, discharge summaries), and bring sound clinical judgment to every visit.

• A current CPR certification and a collaborative, team-first mindset round out the role.

Work Schedule

• Full-Time position.
