  • Department/Specialty: Case Management
  • Schedule: Part Time | Days

What You Will Do

Develop care plans for patients based on assessment findings and consideration of needs and preferences.

Assesse complex emotional, psychological and physical issues to understand what the patient requires and how social work can provide support.

Advocate for patients' needs and ensures the medical system respects their rights and caters to their preferences.

Maintain ongoing dialog with leadership and care transition team members to ensure effective implementation and reevaluation of care plan.

What You Will Need

Licensure / Certification / Registration:

One or More of the Following Required

Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.

Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.

Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education

High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Additional Preferences

  • MSW Preferred.

Requirements

Requires licensure as a Social Worker or Licensed Clinical Counselor from the Wisconsin Board. Education requirements include a high school diploma with 2 years experience, an associate degree, or 4 years of applicable experience, with an MSW preferred.
