Key Responsibilities
Coordinate the interdisciplinary plan of care for patients from admission through discharge while integrating patient and family preferences. Monitor patient care through assessments and medical records to ensure appropriate service delivery and effective health plan implementation.
Requirements
Requires licensure as a Clinical Counselor or Social Worker from the Illinois Department of Financial and Professional Regulation. Candidates must hold at least a high school diploma with 2 years of experience or an associate degree.
