Job Description
Plans, coordinates, and carries out the activities of the (1) Self-Referral
Mammography Program. (2) Recommended Patient follow-up (ACR 0, ACR 3, ACR 4, 5
and 6). (3) Provides breast health information education and support to patients
and families through breast care diagnostic studies process (4) Assists Breast
Cancer Nurse Navigator. (5) Patient and Community Education. The Nurse Breast
Health Navigator is a proficient nurse who has an in-depth knowledge of nursing
practice and takes responsibility for the development and nurturing of self
while enhancing the Breast Health program. This position will include
accountability for maintaining high standards of patient care and patient
education. Work is carried out in an independent, professional manner with
minimal supervision.
Required Knowledge & Skills
Education: Associate Degree or 2 years of college coursework
Experience: One year of related experience. Requires a working knowledge of
standard practices and procedures.
Licenses and Certifications
*GEORGIA REGISTERED NURSE LICENSE OR MULTISTATE NURSING LICENSE
*HEALTHCARE PROVIDER (CPR)
Qualifications
*Meets and maintains the qualifications for Clinical Nurse II.
*BSN preferred.
*Prefer five years nursing experience.

Functions
Area of Responsibilities
*Demonstrates advanced knowledge related to breast health. Plans, coordinates,
and implements activities related to Self Referral Mammography Program.
*(1) Responsible for coordination of physician recommended patient follow-up
(ACR 0's) for Tanner Health System and maintains appropriate documentation.
*Responsible for coordination of all patients within THS Breast Program with ACR
3, 4, 5 and 6 radiographic results. Coordinates activities for surgical consult,
ultrasound and or MRI guided biopsy.
*(1) Participates in Performance Improvement activities. (2) Is knowledgeable of
and demonstrates adherence to The Joint Commission, MQSA guidelines. (3)
Develops knowledge of, adherence to, and assistance with departmental
accreditation goals.
*Assists Breast Cancer Nurse Navigator. (1) Assists with scheduling
appointments, faxing clinical information, orders, etc., for 3-Day Patients. (2)
Assists with assembly of patient information packets, Courage Kits, etc. (3)
Assists with maintaining patient tracking documentation (patient encounters,
referrals, etc.).
*Serves as a patient advocate and a liaison between the patient and family,
Radiologists and other physicians involved in the patient's care to establish
and asssit with implementation of the patient's plan of care.
*Establishes and maintains a good working relationship with Radiologists.
*Exhibits department and facility mission characteristics with patients,
visitors, family members, co-workers, physicians, and or office staff members,
and others they may encounter as a representative of Tanner Health System,
always presenting exemplary customer service.
*Ensures patient safety, privacy, and dignity to the greatest extent possible.
*Documentation is appropriate and in compliance with hospital policy and
procedure and regulatory agency requirements. Appropriate patient records are
maintained.
*Ability to perform public speaking related to general breast health topics to
small and large groups. Communicates effectively and therapeutically in the
community setting.
*Demonstrates competency and ability to navigate all computer modalities.
*Demonstrates competency in patient care according to the age of the patient and
populations served. adult and geriatric. (a) demonstrates the ability to obtain
and interpret information in terms of the patient's needs. (b) Demonstrates
knowledge of growth and development. (c) Demonstrates understanding of the range
of treatment needed by these patients (age and diagnosis).
*Serves on hospital committees as assigned or requested.
*Makes suggestions for improvement of patient care, processes, or procedures.
*Distinguishes and responds appropriately to disaster and/or emergency
situations.
*Performs other related duties as assigned.
*Provides breast health information education and support to patients families
as they navigate through the breast care diagnostic studies processes.
Compliance Statement
