Case Manager for our Hospice division in the Snohomish County area.

Hospice RN Case Manager – Key Responsibilities
* Provide skilled nursing care and symptom management to terminally ill
patients.
* Develop and manage individualized care plans in coordination with the hospice
team.
* Educate and support patients and families through end-of-life care.
* Administer medications and treatments for comfort and quality of life.
* Coordinate care with physicians, social workers, aides, and other team
members.
* Respond to urgent needs and provide crisis support when necessary.
* Maintain accurate and timely documentation in the EMR.
* Manage a caseload, scheduling and prioritizing visits based on patient needs.
* Participate in interdisciplinary team meetings and plan-of-care updates.
* Ensure compliance with hospice regulations and standards.
Qualifications
* Active Washington State Registered Nurse (RN) license.
* Prior Home Health and/or Hospice experience is strongly preferred.
* Current BLS certification from the American Heart Association.
* Valid driver's license and insured, reliable vehicle.
