The Social Worker is responsible for identifying and intervening in biopsychosocial needs impacting health and well-being through functional guidance, service coordination, referrals, and follow-up on Social Determinants of Health (SDOH). They collaborate with the transdisciplinary team to promote care adherence, service continuity, and equitable access to community resources.
Candidates must possess a Bachelor's degree in Social Work (BSW) from an accredited institution and have one to three years of experience in primary health, community services, social programs, or case management. Required credentials include a current professional Social Worker license in Puerto Rico, active membership in the professional college, and CPR-BLS certification, along with bilingual proficiency in Spanish and English.
