Location: This is a hybrid, primarily mobile role serving skilled nursing facilities across northern West Virginia. Candidates must live in or near Keyser, WV, or within a reasonable commuting distance of assigned facilities.

About the Role

The Mobile Nurse Practitioner (NP) provides comprehensive, patient-centered care to Institutional Special Needs Plan (I-SNP) members residing in skilled nursing facilities.

The NP delivers routine and acute care, supports longitudinal care management, and collaborates closely with facility staff and the interdisciplinary care team.

Responsibilities include conducting Health Risk Assessments, developing individualized care plans, participating in weekly facility interdisciplinary team (IDT) meetings, and coordinating care to address members’ ongoing clinical and functional needs.

This highly collaborative, patient-facing role requires strong clinical judgment, effective communication, and the ability to work independently in a dynamic, field-based environment.

Location & Work Environment

This is a full-time W-2 position, Monday-Friday between 8:00 a. m. – 5:00 p. m. Remote work requires a reliable high-speed internet connection and a dedicated, HIPAA-compliant workspace to ensure the security and confidentiality of patient information and organizational communications.

Key Responsibilities

Deliver comprehensive primary and acute care to I-SNP members residing in skilled nursing facilities, including initial, routine, acute, and transition-of-care evaluations

Assess, diagnose, and manage acute illnesses, chronic conditions, and geriatric syndromes within a medically complex long-term care population

Conduct Health Risk Assessments (HRAs) and develop, implement, and adjust individualized, goal-oriented care plans in collaboration with members, families, facility staff, and the interdisciplinary care team

Participate in weekly facility interdisciplinary team (IDT) meetings to review member needs, clinical concerns, care plans, and opportunities to improve outcomes and coordination of care

Order, interpret, and act on laboratory and diagnostic results; prescribe medications and therapeutic interventions in accordance with regulatory requirements and evidence-based clinical practice

Provide proactive follow-up and longitudinal management of a dedicated patient panel, monitoring clinical status, addressing changes in condition, and helping prevent avoidable complications and hospitalizations

Oversee and coordinate transitions of care, including admissions, readmissions, hospital discharges, and changes in level of care, in collaboration with facility staff and the broader care team

Collaborate closely with nursing, rehabilitation, pharmacy, specialists, social services, facility leadership, and other members of the interdisciplinary team to support comprehensive member care

Recognize and respond to early signs of clinical deterioration and changes in condition; provide appropriate treatment, escalation, education, and supportive or palliative care when indicated

Maintain timely, accurate, and compliant clinical documentation in the electronic medical record and adhere to I-SNP program requirements, quality standards, and performance metrics

Work independently in a field-based environment while maintaining strong communication and collaboration with facility partners and the Avail clinical team

Qualifications

Active, unrestricted West Virginia NP and RN licensure and WV DEA registration (or willingness to obtain before start)

• 1–3 years of NP experience, ideally in geriatrics, long-term care, primary care, mobile, or value-based care
• Familiarity with risk adjustment, HEDIS/STARs, and care transitions preferred
• Comfortable working independently, managing time across multiple facilities, and using EHR and telehealth tools
• Strong clinical judgment, assessment, and diagnostic capabilities for complex, high-acuity patient populations

Residence in West Virginia with ability to travel within the assigned region; authorization to work in the United States

Compensation & Benefits

Salary: From $125k to $150k annually, commensurate with experience.

Technology and Equipment Provided

Additional Requirements

• Valid driver's license, reliable transportation, and active auto insurance
