Location: This position is a hybrid, primarily mobile role and must be located within the geographic region of Keyser, West Virginia and surrounding areas, including Baker or within a reasonable commuting distance.

About the Role
The Mobile Nurse Practitioner (NP) provides comprehensive, patient-centered care to Institutional Special Needs Plan (I-SNP) members residing in skilled nursing facilities, delivering routine visits, acute evaluations, and longitudinal care coordination in collaboration with an interdisciplinary team. This highly collaborative, patient-facing role includes conducting Health Risk Assessments and developing individualized care plans, and requires strong clinical judgment, effective communication, and comfort working independently in a dynamic, field-based environment.
Location & Work Environment
This is a part-time W-2 position, scheduled 2 days per week, between 7:00 a. m. – 6:00 p. m. Weekend availability is welcomed. Mobile shifts may vary depending on need. Remote work requires a reliable high-speed internet connection and a dedicated, HIPAA-compliant workspace to ensure the security and confidentiality of patient information and organizational communications.
Key Responsibilities
Deliver comprehensive primary and acute care to I-SNP members in skilled nursing facilities, including initial, routine, and transition-of-care evaluations
Assess, diagnose, and manage acute illnesses, chronic conditions, and geriatric syndromes in a medically complex population
Develop, implement, and adjust individualized, goal-oriented care plans in collaboration with patients, families, and the interdisciplinary team
Order, interpret, and act on labs and diagnostics; prescribe medications and therapeutic interventions in accordance with regulatory standards and evidence-based practices
Provide proactive follow-up to monitor clinical status, prevent avoidable complications or hospitalizations, and manage a dedicated patient panel across long-term care settings
Oversee transitions of care (admissions, readmissions, discharges) and collaborate with nursing staff, rehabilitation services, specialists, and facility leadership
Recognize and respond to early signs of clinical deterioration; provide patient, family, and caregiver education, and deliver palliative or supportive care as appropriate
Maintain accurate, compliant documentation in the electronic medical record and adhere to I-SNP program requirements and quality metrics
Qualifications
- Master's or Doctoral degree in Nursing (MSN or DNP) from an accredited program
- Current national certification as a Nurse Practitioner (FNP or Adult-Gerontology NP)
Active, unrestricted West Virginia NP license and West Virginia DEA registration with prescriptive authority (or willingness to obtain prior to start date), maintained in good standing throughout employment
Minimum of 1–3 years of clinical experience, ideally in geriatrics, long-term care, institutional, mobile care, or value-based care settings; experience with medically complex or frail elderly populations preferred
- Familiarity with risk adjustment, HEDIS/STARs measures, and care transitions best practices preferred
- Strong clinical judgment, assessment, and diagnostic capabilities for complex, high-acuity patient populations
Comfort with remote work tools, EHR platforms, and technology-enabled care delivery; strong communication and organizational skills
Residence in West Virginia with ability to travel within the assigned region; authorization to work in the United States
Compensation & Benefits
- Hourly Rate: Up to $73.00 per hour, commensurate with experience and qualifications.
- Mileage Reimbursement: $0.725 per mile for travel exceeding 30 miles per day, up to $200 per day.
Support and Resources: Avail Health provides all necessary devices for clinical and technology-related activities. Professional liability and malpractice insurance coverage ($1M/$3M) provided..
Retirement: 401(k) retirement plan with a 3% employer match, subject to plan terms and eligibility requirements.
Additional Requirements
Valid driver's license, reliable transportation, and active automobile insurance; must be able to travel within the assigned region and provide proof of insurance upon request
Ability to meet credentialing, privileging, and background check requirements (engagement contingent upon completion)
Compliance with all applicable federal, state, and organizational regulations governing clinical practice
