Opportunity Information Sutter East Bay Medical Group (SEBMG) is recruiting for a Board Eligible or Board-Certified Ambulatory Palliative Care Physician. Provide Palliative Medicine consultation services and participation in Palliative Supportive Care Services interdisciplinary team.

Key Responsibilities

• Provide Palliative Medicine consultation services in multiple settings: Home, SNF, RCFE, Hospital and Office (Palliative Care, Cancer Center, and embedded primary care clinics).

• Coordinate with interdisciplinary team members - each team has an NP, licensed clinical social worker, RN, chaplain, and care coordinator.

• Facilitate access to appropriate supportive care services.

• Coordinate care with inpatient palliative care colleagues, referring providers, outside agencies and local hospices.

Requirements

• Fellowship in Hospice and Palliative Care Medicine.

• Board-certified or board-eligible in Hospice and Palliative Care Medicine.

• CA physician license or eligible for CA physician license.
