Job Description
Make a meaningful difference for patients and families when compassionate support matters most. The Hospice Social Worker provides emotional support, coordinates resources, and advocates for individuals facing life-limiting illnesses. Working under the supervision of an MSW and alongside the interdisciplinary team, the BSW helps patients and caregivers navigate the social, emotional, and practical challenges of end-of-life care.
As a Hospice Social Worker, You Will
- Assess patient and family psychosocial needs, support systems, coping abilities, and barriers to care.
- Collaborate with the supervising MSW and interdisciplinary team to develop and update individualized plans of care.
- Provide supportive counseling within your scope of practice to help patients and families cope with illness, loss, and caregiving responsibilities.
- Connect patients and families with community resources, financial assistance, transportation, and other support services.
- Assist with advance care planning, advance directives, and funeral planning while respecting patient preferences.
- Identify caregiver stress, safety concerns, and changes in family needs, and communicate concerns promptly.
- Participate in interdisciplinary group meetings and coordinate services with nurses, physicians, chaplains, and other team members.
- Support bereavement assessments and referrals in collaboration with the bereavement team.
- Complete accurate, timely documentation according to agency guidelines.
- Protect patient confidentiality and honor each family’s cultural, spiritual, and personal values.
Requirements
- Bachelor of Social Work (BSW) from a program accredited by the Council on Social Work Education.
- Current social work license or other credentials as required by the state of practice.
- At least one year of social work experience in a healthcare setting.
- Ability to provide services under the supervision of a qualified MSW.
- Hospice, home health, or palliative care experience preferred.
- Strong communication, organizational, and problem-solving skills.
- Compassionate approach to working with patients experiencing serious illness and families facing grief and loss.
- Valid driver’s license, reliable transportation, and current auto insurance for patient visits.
- HomeCare HomeBase experience preferred.
