The Physical Therapist provides direct physical therapy services, assesses patient needs, and develops individualized care plans in a home health setting. They also supervise physical therapy assistants and home health aides while maintaining open communication with the interdisciplinary care team.
Candidates must hold a current unencumbered state professional license and active CPR certification. One year of experience as a home care physical therapist and proficiency in OASIS assessments are preferred.
