Key Responsibilities
The Registered Nurse Case Manager is responsible for the proactive coordination and transition of patients to appropriate levels of care to optimize length of stay and resource utilization. They collaborate with multidisciplinary teams to assess patient needs, manage discharge planning, and prevent unnecessary readmissions.
Requirements
Candidates must hold a current Registered Nurse (RN) license and a Bachelor's degree in nursing, or be actively pursuing one. A minimum of three years of acute hospital care experience is required.
