Job DetailsJob Location: Amberwell Hiawatha - Hiawatha, KS 66434Education Level: 4 Year DegreeJob Shift: PRN - As Needed, no set ShiftJob Category: Rehabilitation Services - PT - OTPosition Summary Provide health care services to individuals of all ages who have impairments, functional limitations, disabilities or changes in physical function and health status resulting from injury, disease, or other causes.
Interacts with health care professionals in completing documentation, such as evaluations, care plans, daily notes, and discharges, in a timely manner for all patients. Treat inpatients and outpatients.
Qualifications
Education High School or GED 4 year/Bachelors Degree Post Graduate Degree BLS Registered Physical Therapist with current Kansas License or eligibility Graduate of accredited Physical Therapy Program Current valid driver's license and current proof of insurance Complete continuing education as required by the state Experience None Skills Organizational Verbal Interpersonal Customer Relations Analytical Grammar/Spelling Read/Comprehend written instructions Follow verbal instructions Computer Age of Patients Served Neonate/Infant (Birth-1 year) Child/ PED (1-11 years) Adolescent (12-17 years) Adult (18-64 years) Geriatric (65+ years) Mental and Emotional

Requirements

Key Responsibilities
Provide healthcare services to individuals of all ages with physical impairments or disabilities resulting from injury or disease. Complete clinical documentation including evaluations, care plans, and discharge notes for both inpatients and outpatients.
Requirements
Requires a bachelor's degree, a postgraduate degree, and a current Kansas Physical Therapist license. Must be BLS certified and possess a valid driver's license with proof of insurance.
