The Physical Therapist will perform home health visits to conduct diagnostic tests, develop individualized care plans, and implement therapeutic interventions for adult and geriatric clients. They are also responsible for educating clients and families while accurately documenting care using electronic medical records.
Candidates must hold a current New Hampshire Physical Therapist license and have at least one year of recent clinical experience. A degree from an accredited physical therapy program is required, along with strong organizational and interpersonal skills.
