Behavioral Health Counselor

• Part-Time: 2-3 days per week, with Potential to Become Full-Time
• Hybrid: scheduled onsite days each month in Engelhard and Manteo
• Pay: $32–$36 per hour

Ocracoke Health Center, Inc. is seeking a Behavioral Health Counselor / Integrated Care Program Lead to provide behavioral health services as part of our Patient Centered Health Team. This position offers a combination of remote work and scheduled onsite days at our Engelhard Medical Center and Manteo Community Health Center locations.

The position is hybrid and will initially be part-time, approximately two to three days per week. We are flexible in establishing the onsite schedule and will work with the selected candidate based on their availability and patient needs.

We are also pursuing grant funding to expand behavioral health services, allowing this position to be full-time. Full-time employment is contingent upon funding and organizational needs. If expanded to a full-time position, employees are eligible for a comprehensive benefits package that currently includes medical, dental, vision, short-term disability, long-term disability, life insurance, and paid time off (PTO).

What You’ll Do

• Provide evidence-based behavioral health interventions within a primary care setting.
• Assess patients referred by primary care providers and develop patient-centered care plans.

Use behavioral health screening and assessment tools to identify concerns, support triage, and measure outcomes.

Collaborate with medical providers and other members of the care team to support patients with emotional, behavioral, and mental health needs.

• Refer patients to specialty behavioral health, case management, or community resources when appropriate.
• Maintain accurate and timely documentation in the electronic health record – we use Athena
• Participate in chart reviews, quality improvement initiatives, clinical guideline development, staff education, and Patient-Centered Medical Home collaboration.
• Provide guidance and supervision to the Behavioral Health Case Manager.

Requirements

Master’s degree from an accredited program in social work, psychology, marriage and family therapy, counseling, behavioral health, or a closely related field.

• Current North Carolina licensure as one of the following:
• Licensed Clinical Social Worker, LCSW
• Licensed Marriage and Family Therapist, LMFT
• Licensed Clinical Mental Health Counselor, LCMHC
• Strong communication, clinical assessment, documentation, crisis intervention, and care coordination skills.
• Ability to build effective working relationships with patients, providers, team members, and community partners.
• Experience in integrated care, primary care, community health, or rural behavioral health is a plus.
• Leadership or supervisory experience is preferred, but not required.
• Spanish-speaking ability is a plus.
