The therapist will perform comprehensive diagnostic assessments and provide patient visits within long-term care facilities. They are also responsible for maintaining up-to-date treatment plans and collaborating with psychiatric and case management teams.
Candidates must hold a Master's degree in social work or professional counseling and possess an active LCSW or LMHC license. A valid driver's license and the willingness to travel within the assigned service area are also required.
