Key Responsibilities
The RN Case Manager conducts initial and ongoing patient assessments to develop and revise individualized care plans. They provide direct nursing care, administer medications, and coordinate discharge planning while counseling patients and families.
Requirements
Candidates must hold a current state RN license and have at least one year of recent acute care experience. A valid driver's license, auto insurance, and BLS/CPR certification are required.
