Conduct thorough patient assessments to determine home care needs and develop individualized care plans. Provide comprehensive physical assessments, document illness history, and manage discharge planning needs.
Requires a current and valid state RN license, at least one year of recent acute care experience, and BLS/CPR certification. A valid driver's license and auto insurance are mandatory, with home health experience preferred.
