Remote RN Case Manager - Full Time

Key Responsibilities

The Remote RN Case Manager will manage provider panels, coordinate follow-up care, and provide patient education to support self-management of chronic conditions. They will also collaborate with a multidisciplinary team to update care plans and provide clinical guidance to back-office staff.

Requirements

Candidates must hold an active Registered Nurse license in the State of Washington and possess at least two years of recent clinical or case management experience. A reliable, high-speed internet connection and proficiency with Electronic Health Record systems are also required.
