Key Responsibilities

up visits and care adjustments based on findings

Conduct comprehensive evaluations: o Gather medical histories and perform endocrine

focused physical exams o Order and interpret labs including A1C, thyroid panels, metabolic panels, bone density tests, and other diagnostics

Collaborate with endocrinologists, interdisciplinary teams, nursing staff, and facility providers to develop treatment plans, manage complex metabolic conditions, and reduce hospital readmissions

Requirements

Minimum of 6 months of experience in a long

term care or skilled nursing facility (Hospital/outpatient office experience will also be considered)

Active and unrestricted NP license

DEA license required

Work Schedule

Full-time position.
