Key Responsibilities

  • up visits and care adjustments based on findings
  • Conduct comprehensive evaluations: o Gather medical histories and perform endocrine
  • focused physical exams o Order and interpret labs including A1C, thyroid panels, metabolic panels, bone density tests, and other diagnostics
  • Collaborate with endocrinologists, interdisciplinary teams, nursing staff, and facility providers to develop treatment plans, manage complex metabolic conditions, and reduce hospital readmissions

Requirements

  • Minimum of 6 months of experience in a long
  • term care or skilled nursing facility (Hospital/outpatient office experience will also be considered)
  • Active and unrestricted NP license
  • DEA license required
