About the Role

We’re looking for a compassionate and skilled Physical Therapist to join our growing home health team. In this role, you’ll have the opportunity to make a meaningful impact by helping patients regain independence and improve their quality of life, all from the comfort of their homes or where they reside.

If you’re someone who values autonomy, flexibility, and building strong patient relationships, this is a great opportunity to thrive in a supportive, collaborative environment.

  • Develop and implement personalized treatment plans based on patient needs and physician orders
  • Guide patients through therapeutic exercises, gait training, balance and coordination activities, and mobility improvement
  • Educate patients and caregivers on safety, fall prevention, and daily living strategies
  • Monitor patient progress and adjust treatment plans to achieve optimal outcomes
  • Assess vital signs as needed and communicate any concerns to the care team
  • Recommend and educate on the use of assistive devices and durable medical equipment (DME)
  • Collaborate with an interdisciplinary team including nurses, occupational therapists, speech therapists, and physicians
  • Complete timely and accurate documentation
  • Support discharge planning and ensure continuity of care
  • What We’re Looking For
  • Licensed Physical Therapist in the state of Massachusetts
  • Degree from an accredited Physical Therapy program
  • CPR certification (current)
  • Valid driver’s license and reliable transportation
  • Home health experience required
  • Strong communication, organization, and clinical decision-making skills
  • Preferred
  • At least 1 year of clinical experience

Why Join Us?

  • Autonomy and flexibility in your daily schedule
  • Meaningful, one-on-one patient care
  • Supportive and collaborative team environment
  • Opportunities for professional growth and continuing education
