Job Description
Provides in-home evaluation, direction, and application of Physical Therapy services to relieve pain, develop or restore function, and/or maintain maximum physical performance of patient.
Education
Required: Graduate of approved program in Physical Therapy as referenced in Conditions of Participation 54 FR 33354 (@484.4), or Two years experience as Physical Therapist and satisfactory grade on proficiency exam conducted, approved, or sponsored by U.S. Public Health Service. (N/A if licensed or sought qualification as PT after December 31, 1977).

Required: At least 5 years’ of related experience
Skills
Ability to provide reliable transportation as required by nature of position.
Licensure/Certification/Registration
- Required: Current Physical Therapist license or registration as required by state
- Required: Active Driver's License
Key Responsibilities
Provides in-home physical therapy evaluations and treatments to help patients restore function and manage pain. Develops and applies therapeutic plans to maintain or improve the physical performance of patients.
Requirements
Requires a degree from an approved physical therapy program and at least 5 years of related experience. Must hold a current physical therapist license and a valid driver's license with reliable transportation.
