- Provide rehabilitative services to patients under specific medical orders as part of a coordinated team.

Position Requirements
Degree in Area of Discipline
- Current Physical Therapy license in state of practice, or ability to obtain prior to beginning employment
- Excellent interpersonal, fiscal, and clinical skills

Pay Range
USD $45.00 - USD $50.00 /Hr.

Provide individualized rehabilitative services to senior patients under medical orders within a coordinated team. Plan and execute treatment programs based on clinical findings while participating in care coordination conferences.
Requirements
Requires a degree in physical therapy and a current physical therapy license in the state of practice. Candidates must possess excellent interpersonal, fiscal, and clinical skills along with computer proficiency.
