Job Description

Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. What you’ll do as a Physical Therapist PT

Conduct comprehensive evaluations and develop individualized treatment plans that empower patients to reach their maximum potential.

Implement evidence-based therapeutic interventions, including manual therapy, therapeutic exercises, and modalities.

Monitor patient progress, adjust treatment plans as needed, and ensure timely and thorough documentation.

Educate patients and their families on treatment options, rehabilitation techniques, and preventative measures.

  • Collaborate with physicians and other healthcare professionals to ensure a holistic approach to patient care.
  • Help create a welcoming and supportive clinic environment where patients feel comfortable and motivated.

Who you are as a Physical Therapist PT

  • A graduate of an accredited Physical Therapy program with a valid state license.
