The Nurse Practitioner will provide home-based palliative symptom assessment, treatment, and ongoing goals of care discussions for patients with advanced illnesses. They will work within an interdisciplinary team to manage chronic disease states and facilitate education for patients, families, and the clinical team.
Candidates must hold a Master's degree in a Nurse Practitioner program and possess a current New York State license and registration. A minimum of 3 years of palliative care or geriatrics experience and national board certification in advanced practice nursing are required.
