Home Health Physical Therapist

Join Halcyon Home as a Home Health Physical Therapist in Spring, Texas. Halcyon Home is seeking a dedicated and compassionate Home Health Physical Therapist PRN to support our growing geriatric home health population in Tomball/Spring, TX and the surrounding area. Key Responsibilities

• A minimum of one year of home health experience is preferred, along with a proven ability to manage a fast-paced, independent work schedule. Knowledge of Homecare Homebase software is a plus!

• Performs ongoing clinical assessment, including OASIS assessments, at appropriate time points to identify patient needs, determine level of functioning, and appropriateness of patient for home care.

• Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver(s).

• Documents, prepares, and completes clinical notes, phone orders, and other clinical record documentation on an ongoing basis and in a timely manner. Revises the POC based on evaluation and ongoing assessment data.

Requirements

• Current PT license.

• Knowledge or experience utilizing Homecare Homebase EMR would be a plus.

• Experience in home health or related healthcare setting preferred.

• Strong clinical and assessment skills.

Benefits & Compensation

• Pay rate: $75 - Pay rate50/hour.

Work Schedule

• Per Diem position.
