Core Job Summary

The Specialty Pharmacist 2 (H) independently provides comprehensive clinical and operational pharmacy services for patients receiving specialty medications through UHealth Specialty Pharmacy. The pharmacist is responsible for safe and effective medication use, prescription verification, patient education, therapy monitoring, clinical interventions, medication access support, and coordination with prescribers and interdisciplinary care teams while supporting specialty pharmacy quality, accreditation, regulatory compliance, patient retention, and operational performance across the medication-use continuum.

Core Job Functions

1. Independently provides comprehensive clinical and operational pharmacy services for patients receiving specialty medications through UHealth Specialty Pharmacy.

2. Reviews and verifies specialty medication prescriptions for appropriateness, accuracy, dose, indication, allergies, drug interactions, contraindications, laboratory parameters, and applicable clinical requirements.

3. Provides patient education and counseling regarding specialty medications, including administration, adherence, adverse effects, storage, handling, monitoring, and therapy-specific requirements.

4. Performs initial and ongoing clinical assessments, medication reconciliation, adherence assessments, therapy monitoring, and clinically appropriate follow-up in accordance with specialty pharmacy protocols and accreditation standards.

5. Identifies, documents, resolves, and communicates medication-related problems and clinical interventions with prescribers, nurses, pharmacy team members, and other members of the interdisciplinary care team.

6. Collaborates with medication access, prior authorization, patient assistance, financial assistance, ambulatory pharmacy, clinic, and payer teams to facilitate timely access to specialty medications.

7. Supports prescription capture and patient retention by coordinating specialty prescriptions within UHealth and resolving clinical, access, fulfillment, and continuity-of-care barriers.

8. Ensures specialty pharmacy services comply with applicable federal and state regulations, Florida Board of Pharmacy requirements, accreditation standards, payer requirements, manufacturer requirements, and organizational policies.

9. Maintains complete, accurate, and timely documentation of clinical assessments, interventions, patient communications, therapy outcomes, and required specialty pharmacy activities.

10. Monitors patient response to therapy and relevant laboratory or clinical parameters and provides clinically appropriate recommendations or escalation.

11. Provides drug information and clinical recommendations to patients, prescribers, nurses, pharmacy staff, and other healthcare professionals.

12. Manages specialty medication requirements, including REMS, limited distribution, manufacturer programs, payer requirements, and other therapy-specific controls.

13. Participates in quality improvement, medication safety, patient experience, accreditation readiness, regulatory readiness, and performance improvement initiatives.

14. Precepts and supports the development of pharmacy residents, students, technicians, and other pharmacy team members as appropriate.

15. Adheres to University and unit-level policies and procedures and safeguards University assets.

Education

Bachelor’s degree - Pharmacy (BPharm) required

Experience

Minimum 3 years of relevant experience required

Certification and Licensing

  • Valid State of Florida Registered Pharmacist license required
  • Refer to department description for applicable certification requirements
