Completion of an accredited Oral and Maxillofacial Surgery residency program.

Board certification or board eligibility with the American Board of Oral and Maxillofacial Surgery (ABOMS). Active state dental license with appropriate anesthesia and sedation permits. Current BLS and ACLS certifications.
