The Nurse Case Manager provides field-based nursing care, triage, and health education to individuals experiencing homelessness within a trauma-informed, harm-reduction framework. They collaborate with a multidisciplinary team to manage chronic conditions and facilitate access to supportive housing and health services.
Candidates must hold an Associate, Bachelor, or Master degree in Nursing and possess a current Registered Nurse license in the State of Colorado. Basic Life Support certification is required, and experience in community, mental health, or substance use disorder nursing is preferred.
