Key Responsibilities

The Care Coordinator will conduct outreach and engagement efforts to link clients with behavioral, physical, and social health services. They are responsible for developing individualized recovery goals, managing crisis plans, and coordinating with various providers to ensure successful service delivery.

Requirements
