The coordinator provides integrated, person-centered care for adults with behavioral health needs by collaborating with multidisciplinary teams and community partners. Key duties include conducting assessments, creating individualized care plans, and connecting clients to essential social and medical resources.

Requirements

Candidates must have a High School Diploma or GED and a required MHRT/C certification. A minimum of 2 years of experience in the behavioral health field and a valid driver's license are also required.
