Job Description
The Family Nurse Practitioner (FNP) provides medical care, health assessments, and medication management for residents and clients of Fairview’s Lifestyle Recovery Programs, which deliver ASAM Level 2.1, 3.1, and 3.5 substance use disorder treatment across a three-county area of Southwest Virginia. The FNP collaborates with the treatment team at each location to support safe, effective, recovery-focused care and ensures that all clinical services comply with Virginia licensing requirements and ASAM standards.
Essential Duties and Responsibilities
Clinical Assessment and Medical Care
Complete medical history and physical assessments for new admissions, including review of co-occurring physical and behavioral health conditions, allergies, current medications, and substance use history.
Assess and monitor withdrawal risk and symptoms using standardized tools and clinical judgment, and determine when a resident requires a higher or different level of medical care.
Evaluate and treat acute and chronic health conditions within scope of practice, and order, interpret, and follow up on laboratory and diagnostic testing.
Screen for infectious diseases and other health risks common in this population, and provide education, treatment, or referral as appropriate.
Coordinate emergency transfers and referrals to hospitals, specialists, and community providers when needs exceed program capability.
Medication Management
Prescribe, adjust, and monitor medications, including medications for opioid, alcohol, and other substance use disorders (MOUD/MAT) and psychotropic medications, in accordance with evidence-based practice and program policy.
Review the Virginia Prescription Monitoring Program and follow state and federal requirements for controlled-substance prescribing.
Conduct medication reconciliation at admission, transfer, and discharge; monitor for side effects, adherence, interactions, and diversion risk.
Provide medication orders, guidance, and oversight to nursing and medication-administration staff, and review medication errors or incidents as needed.
Educate residents on medications, risks, benefits, and relapse-prevention supports such as overdose prevention and naloxone.
Treatment Team Collaboration
Participate in multidisciplinary treatment team meetings and contribute medical input to individualized service plans and ASAM level-of-care determinations.
Communicate regularly with clinicians, nursing staff, residential staff, and program leadership at each location regarding resident medical status and needs.
Support continuity of care by coordinating with primary care providers, behavioral health providers, pharmacies, and community resources, and by assisting with discharge and aftercare planning.
Promote a trauma-informed, person-centered, recovery-oriented approach in all resident interactions.
Documentation and Compliance
- Maintain timely, accurate, and complete clinical documentation in the electronic health record in accordance with DBHDS licensing regulations (12VAC35-105), DMAS requirements, and program policy.
- Protect resident confidentiality in compliance with HIPAA and 42 CFR Part 2.
Ensure clinical practice meets applicable Virginia Board of Nursing and Board of Medicine requirements, ASAM criteria, and the standards of any applicable accrediting or payer bodies.
Support infection control, medication storage, and health and safety standards; participate in quality assurance, chart audits, incident reviews, and licensing or payer audits.
Contribute to the development and review of medical policies, protocols, and standing orders.
Travel and Other Duties
- Travel between program locations within the three-county service area as scheduled.
- Participate in on-call coverage or rotation as assigned [if applicable].
- Perform other duties as assigned that are consistent with the role and scope of licensure.
Required Qualifications
- Education: Master's degree or higher from an accredited Family Nurse Practitioner program.
- Licensure: Current, unrestricted license as a Nurse Practitioner in the Commonwealth of Virginia (or a valid multistate license, where applicable) with prescriptive authority.
- Certification: Current national board certification as a Family Nurse Practitioner (AANP or ANCC).
- Federal Registration: Current DEA registration (or ability to obtain prior to start date).
- Practice Authority: Ability to practice in accordance with Virginia requirements, including a practice agreement with a patient care team physician or autonomous practice attestation, as applicable.
- Life Support: Current CPR/BLS certification.
- Clearances: Satisfactory criminal background check, central registry and sanction checks, and TB screening as required by DBHDS regulation; ability to pass a pre-employment drug screen.
- Experience: Two or more years of clinical experience as a nurse practitioner
Skills: Strong assessment, diagnostic, and prescribing skills; working knowledge of ASAM criteria; excellent communication and teamwork skills; proficiency with electronic health records and standard computer applications.
Preferred Qualifications
Experience in substance use disorder treatment, addiction medicine, behavioral health, or residential treatment settings.
Experience with medications for opioid use disorder, withdrawal management, and treatment of co-occurring disorders.
Addiction medicine or behavioral health certification or training (e. g., CARN-AP, addiction-focused CME).
- Familiarity with DBHDS licensing, DMAS/Medicaid requirements, and the Virginia treatment system.
- Experience working with rural and underserved populations.
Knowledge, Skills, and Abilities
- Knowledge of the pharmacology of substance use disorder treatment and psychotropic medications.
- Knowledge of the ASAM Criteria, DSM-5-TR, evidence-based practices, and harm reduction principles.
- Knowledge of HIPAA, 42 CFR Part 2, and Virginia and federal controlled-substance regulations.
- Ability to exercise sound clinical judgment and respond calmly to medical and behavioral emergencies.
- Ability to build rapport with residents in early recovery and communicate in a respectful, nonjudgmental manner.
- Ability to manage time, travel, and documentation across multiple sites.
Physical Requirements and Working Conditions
Work is performed in residential treatment and outpatient clinic settings; regular travel between locations by personal vehicle is required.
Ability to sit, stand, and walk for extended periods; occasionally lift up to [25] pounds; and perform physical assessments.
Exposure to residents with communicable diseases, behavioral crises, and bodily fluids; adherence to universal precautions is required.
Ability to work a flexible schedule, including occasional evenings, weekends, or on-call hours, as needed
