Position Overview
We are seeking a compassionate, dedicated, and experienced Registered Nurse (RN) Case Manager to join our growing hospice team.
The ideal candidate is a skilled nursing professional who is passionate about providing exceptional end-of-life care while supporting patients and families through every stage of the hospice journey.
As a Hospice RN Case Manager, you will independently manage an assigned patient caseload, coordinate individualized plans of care, and collaborate with an interdisciplinary team to ensure patients receive the highest standard of comfort-focused care.
We are looking for a dependable, organized, and clinically strong nurse who values patient advocacy, effective communication, and quality care.
Key Responsibilities
Manage and coordinate hospice care for an assigned caseload of patients in their homes, assisted living facilities, and skilled nursing facilities.
Perform comprehensive nursing assessments, including evaluation of pain, symptoms, functional status, and overall clinical condition.
Develop, implement, and update individualized plans of care in collaboration with the hospice interdisciplinary group (IDG).
Identify, assess, and document changes in patient condition, including clinical decline and disease progression.
Provide skilled nursing interventions focused on symptom management, pain control, comfort, and quality of life.
Communicate regularly with attending physicians, hospice medical directors, patients, families, and caregivers regarding patient needs and changes in condition.
Educate patients and families on disease progression, medications, symptom management, and end-of-life expectations.
Ensure timely, accurate, and comprehensive clinical documentation in accordance with Medicare hospice regulations, agency policies, and applicable standards of care.
Participate in interdisciplinary group meetings and contribute to ongoing care planning.
Coordinate with hospice aides, social workers, chaplains, and other members of the care team to ensure continuity of care.
Recognize and communicate significant clinical changes that may affect hospice eligibility and ongoing care needs.
Provide emotional support and compassionate guidance to patients and their loved ones.
Qualifications
Required
Current, active Registered Nurse (RN) license in the state of practice.
Minimum of one year of Hospice Case Management experience
Attend IDG and Daily standup
Strong clinical assessment, critical thinking, and patient care coordination skills.
Excellent communication, organizational, and time-management abilities.
Ability to work independently and manage multiple patient care priorities.
Valid driver's license, reliable transportation, and ability to travel throughout the assigned service area.
Proficiency with electronic medical records and clinical documentation.
Preferred
Previous hospice or palliative care nursing experience.
Experience in home health, case management, geriatrics, or skilled nursing.
Familiarity with Medicare hospice regulations, documentation standards, and hospice eligibility guidelines.
Experience with symptom management and end-of-life care.
