The Physical Therapist will provide physician-prescribed physical therapy aimed at improving or minimizing residual physical disabilities and returning the individual to their optimum productive level. Responsibilities include initial assessment, plan of care development, modifications, reassessments every 30 days, and participation in patient care planning with other home care personnel.
Candidates must possess a degree in physical therapy from an accredited program and hold a current state license to practice. Preferred qualifications include one year of experience in a home health setting and knowledge of Medicare OASIS documentation.
