Job Summary
Hospice of South Texas (HOST) is seeking a compassionate Medical Social Worker to support patients and families through end-of-life care. This role is open to LBSW, LMSW or LCSW candidates with at least one year of healthcare experience.

Essential Functions
- Complete psychosocial assessments for hospice patients and families
- Provide emotional support, counseling, and resource coordination
- Participate in interdisciplinary team meetings
- Assist with care planning, coping strategies, and end-of-life support
- Maintain timely and accurate clinical documentation
- Collaborate with nursing, chaplaincy, and bereavement teams
Qualifications
- Required
- LBSW, LMSW or LCSW (Texas)
- Minimum of one (1) year of experience in a healthcare setting (regulatory requirement)
- Strong communication and interpersonal skills
- Ability to manage emotionally complex situations with professionalism
Preferred
Experience in hospice, home health, hospital, SNF, dialysis, oncology, or behavioral health settings
Work Schedule: Full-time
Community-based hospice care with defined service territory

Key Responsibilities

Licensure & Certifications
Candidates must hold an LBSW, LMSW, or LCSW license in Texas and possess at least one year of experience in a healthcare setting. Strong communication skills and the ability to manage emotionally complex situations are required.
