Key Responsibilities
Provides counseling, support, and transition planning for patients and their families to address non-medical needs during hospitalization. Collaborates with medical staff and outside agencies to facilitate effective discharge plans and post-acute care.
Requirements
Requires a Master's degree in Social Work and active LSW or LCSW licensure. Candidates must also hold BLS certification and have completed Techniques for Effective Aggression Management (TEAM) training.
