The Patient Navigator RN provides guidance and support to patients and families throughout their care continuum, facilitating communication and coordinating services. They assess patient needs, address barriers to care, and collaborate with interdisciplinary teams to optimize treatment outcomes.
Candidates must have 3-5 years of clinical nursing experience and hold a valid RN license. A Basic Life Support (BCLS) certification is also required for this position.
