Hospice RN Case Manager

Key Responsibilities

The RN Case Manager will conduct ongoing physical and psycho-social assessments of patients and families while developing collaborative plans of care. They are responsible for providing symptom management education, ensuring necessary supplies are available, and maintaining accurate documentation.

Requirements

Candidates must hold an active, unrestricted Registered Nurse license in the State of Michigan and possess at least one year of relevant RN experience. A valid driver's license, reliable transportation, and current CPR certification are also required.
