Join Interim HealthCare as a Hospice RN Case Manager – Flexible, ESOP in ALLENDALE, MI, US. Interim HealthCare of West Michigan is a locally owned and operated leader in post-acute care services in West Michigan. We are now hiring a Registered Nurse within our Hospice division.

Key Responsibilities
Conduct ongoing physical and psycho-social assessments of patients and their families to develop and revise collaborative plans of care. Provide education and interventions for pain management while ensuring all necessary medical supplies and documentation are maintained.
Requirements
Requires an active, unrestricted Michigan RN license, current CPR certification, and at least one year of relevant RN experience. A valid driver's license, reliable transportation, and experience in home health or management are preferred.
